The Missouri Academy of Family Physicians

Founded in 1947, MAFP is the only state-level medical society devoted solely to primary care and a state chapter of the American Academy of Family Physicians, the largest medical society in the country.  The MAFP represents nearly 1700 physicians and medical students across Missouri.  Nearly one in four office visits are made to family physicians. That is 207 million visits annually - nearly 62 million more than the next specialty! Family physicians provide the majority of care for America's underserved and rural populations. Through this site, we hope to provide useful information and resources regarding primary care in Missouri.

PCMH Legislative Definition: 
A " Medical Home" will provide enrolled patients a continuing relationship with a broadly trained Personal Physician.  Care in the "Medical Home" is comprehensive and patient centered, including first contact care, appropriate preventative care, and care of chronic conditions with coordination of services across the entire health care spectrum including end of life care.  To improve outcomes, care in the "Medical Home" will be evidence based and readily accessible.
